1.  What information is collected on the Site?

Collection of Your Personal Information

We collect personally identifiable information (e.g., name, address, telephone number, e-mail address, date of birth, gender, social security number) that you knowingly provide through our Site.  For example, you may Opt Out or Opt In to participating in the HealthInfoNet System within the Site.  To do so, we request your full name, address, date of birth, gender, telephone number, e-mail address, and social security number (SSN is optional).  We use this information to respond to your Opt Out or Opt In request in accordance with this Privacy Policy.  Other pages on this Site may allow you to provide us with personal information to express interest in HealthInfoNet programs, ask a questions or request additional information.   

Information Sent to Us by Your Browser

Certain information about your computer hardware and software is automatically collected when you use the Site.  Such information may include: your IP address, browser type, domain names, pages within our Site that you visit, how long you stay on our Site and pages, the identity of your Internet Service Provider, the name and version of your operating system and referring sites. This information cannot be used to personally identify you unless you identify yourself to us.  We collect this information to learn what pages and content are most valuable to our users and to improve the Site.

With respect to personal information collected on the Site, HealthInfoNet uses such information to provide users with the services they have requested.  For example, Personal information entered into one of our secure forms, which includes our HealthInforNet System Opt-out form and Opt-in forms and our audit request form is used to process your request as set forth in the applicable form.

If you have provided us with personal information, we may use this to make you aware of new services HealthInfoNet is offering or to ask your opinion on surveys.

3.  Does HealthInfoNet share information with third parties?

We may share your information with trusted third party service providers to help us with management and operation of the Site, communications activities, web analysis, customer support and other important services. 7.  Governing Law.

This Privacy Policy will be governed by and construed in accordance with the laws of the State of Maine, without giving effect to its conflicts of law provisions. By accessing or using the Site, you consent and submit to the exclusive jurisdiction and venue of the state and federal courts located in Maine.  A printed version of this Privacy Policy will be admissible in judicial and administrative proceedings based upon or relating to this Privacy Policy to the same extent and subject to the same conditions as other business documents and records originally generated and maintained in printed form.
